Tracheal Agenesis: Rare but Challenging

Our patient was a baby boy born term by normal vaginal delivery, without any apparent external abnormality (Weight: 3Kg; Height: 50 cm; Head circumference: 35.5cm), to a 27 year old mother G2 P1, non consanguineous parents, pregnancy complicated by polyhydramnios, with an APGAR score of 0 and 8 at 1 and 5 minutes respectively. He was intubated and ventilated (difficult intubation). However, he remained to have desaturations with serial arterial blood gases constantly showing CO2 retention (Arterial blood gas: pH=7.13, CO2=84, HCO3=26, pO2=95) despite adequate ventilator settings (CMV mode: RR=50, PIP=18, PEEP=3.5, FiO2=21%).
